The Academic Evaluation should be completed by a teacher or guidance counselor who you would like to serve as your reference. Enter your information, as well as your reference's information and they will be sent an electronic version of the Academic Evaluation form. (Home school students can use this form if they have a teacher, other than a parent, who can complete it. Otherwise, we ask for a reference letter from someone who is not a family member.)
